Pick one you actually owned
A real call you made, not a project someone else sank. A safe answer reads as evasion.
Say what went wrong in one line
Enough for them to understand the stakes, no more. The failure is not the interesting part.
Spend the answer on what you changed
This is the whole question. What you do differently now, and why.
End on the result of the fix
Evidence the change stuck. Not an apology.
What most people get wrong: they pick a failure that is secretly a success, or one where somebody else was at fault. Interviewers hear it instantly, and it turns a question about judgement into a question about honesty.
